How we handle your information.

Puget Sound Eye Care collects only what is necessary to deliver clinical care and operate the practice — patient demographics, insurance information, exam findings, and the answers you provide on appointment, contact, and recall forms. Protected health information (PHI) is governed by HIPAA.

What we collect

  • Information submitted through appointment-request and contact forms
  • Insurance card, plan, and member ID for benefits verification
  • Clinical examination data, refraction, ocular health, and prescription records
  • Aggregate, non-identifying analytics on site traffic and performance

How we use it

Information is used to deliver care, process claims with your insurance carrier, communicate appointments and recalls, and improve the patient experience. It is never sold. PHI is shared only with the entities required to deliver care (referring physicians, contact-lens labs, insurance carriers) and only with patient authorization or as permitted by HIPAA.

Your rights under HIPAA

You have the right to inspect and copy your medical record, request corrections, request an accounting of disclosures, and request restrictions on uses. Requests should be directed to the front desk at either office, in writing, and will be acknowledged within thirty days.
